110-300-0165(2)(c) Licensee had a white rope material hanging from the toddler window that could form a loop around a child neck. 110-300-0165(2)(d) Licensee had plastic bags accessible in the infant room under the changing table, in the toddler room on top of the cubbies, and in the toddler bathroom under the changing table. 110-300-0165(3)(f) Licensee had two sets of cubbies in the preschool room that were not anchored, and a large water holder in the preschool room on top the of cubbies, and in the toddler room on top of the cubbies that were not anchored posing a risk of tipping or falling. 110-300-0165(3)(g) Licensee had flaking paint in toddler room in the window ceils, walls by the bathroom door, inside the door jamb of the bathroom toddler room door on both sides. Licensee has a torn tarp over a play structure in the outdoor play area that is damaged and torn that needs to be removed from the licensed space. 110-300-0165(4)(e) Licensee had a water temperature of 125.3 Fahrenheit during this onsite visit.
